Getting ready for your first plan meeting

Updated 2026-10-07. General information, not legal or benefits advice.

The Individual Service Plan, or ISP, is the plan that lists the supports a person needs and wants. It is built with your Service Coordinator and reviewed at least once a year. A little preparation makes the meeting much easier.

Who is usually there

  • The person receiving services. The plan is about them, so their voice comes first.
  • Family members and the guardian, if there is one.
  • The Service Coordinator (SSA).
  • Current providers, when the person wants them there.

What to bring

  • A short description of a typical week: what goes well and what is hard.
  • Goals for the next year, in the person's own words when possible.
  • A list of current supports, doctors, and medications.
  • Any concerns about safety, health, or behavior.
  • Providers you are interested in. Your meeting sheet from this site works well here.

Questions worth asking

  • Which services can be added to the plan, and how is funding decided?
  • What happens if the provider we want has no openings?
  • How long after the meeting can services start?
  • Who do we call between meetings if something changes?

You can ask for a copy of the plan. Read it before you sign, and ask for changes if something does not match what was discussed.
